Students consistently causing same problems in all the courses they have taken

We do have a faculty/student liaison, though, who is the primary arbiter of these kinds of disputes, so it would have been escalated to her, first. She is a pretty good de-escalator; basically she lets students talk until they feel heard, then sorts through the evidence and decides where to direct it next - it can go to the chair, and then on to the dean if the student insists, but she can usually mediate it so that it doesn't go that far. I wouldn't have her job for anything, but she does it very well. That's not ALL she does, of course (we don't have that many issues, thank goodness) but it's also part of her job.

Thanks for all the advice guys. Just a heads up:

I got the students' evaluations today. All of them were positive except for one that was really negative. Funny thing: after what happened, I think I know who was the student who gave me this really bad evaluation....
