How-To Guide on Interviewing

I'm going to be mentoring a graduate student whose research involves interviewing subjects. The process will probably begin with a standard set of simple survey-type questions but evolve into semi-structured interviews. I know what I do for this kind of data collection but don't have a handy reference tool to provide to the student. Any suggestions?
It's terrible writing, used to obfuscate the fact that the authors actually have nothing to say.
