When is it appropriate to begin using new title?

I would go ahead and update.  Sou da like in this case it is saving the IRB extra work.  If I were one of them, I would appreciate that, although I wouldn't notice.

The truth here, is it really doesn't matter.  I would be shocked if anyone on the IRB would care.  Where I am, only the people on the T&P committee would even know any details, and they are likely not on the IRB. 

Even if someone noticed, and even if they cared, what are they going to do?  You have tenure!

I waited to switch titles on grant applications since it was not my official title yet, but this is not a case where it is under review so I don't see any issue.

but in the next few weeks I'll be submitting IRB amendment for a study with 8 consent forms, which will all need to be revised with my new title. Is it appropriate to just make the change with this submission? It's easiest on both ends (I doubt the IRB wants another amendment) but I also don't want to do this if it would be viewed as inappropriate. Thoughts are appreciated.

Agreed. However, it often feels like there are unspoken rules in academia and SOME judge harshly if you are unaware (and just as harshly is you are aware and still choose to defy). What I appreciate about this forum is that I can anonymously ask a question I fear could receive an "how do you not know this?" response (one way or the other) without fear that I'll forever become that person who didn't know X... (except perhaps in this forum). Again, everyone's responses and suggestions are much appreciated!

On a CV you can put new title and a start date. In email, don't use new title until start date or very close.
When on doubt, stick to what you can back up officially.

To frame this issue more generally regarding university support units. If you have a good organization, and are reasonably kind, the people in the support units are usually super receptive to faculty asking how things work. Test it at your own place, but I think you'll find that a safe space for revealing ignorance about procedural minutia. You can do it in a way that shows respect for their expertise and for the role they play in making your work effective.
